In short: A daily OPD register has one row for every patient visit. The essential columns are serial number, OPD or token number, registration number (UHID), name, age and sex, mobile or city, new or old, chief complaint, diagnosis, treatment, doctor, fee and next visit. At the end of the day, total the new and old patients, male and female, Panchakarma advised and fees collected.
The OPD register is the simplest record in a clinic, and the one most often kept badly. Columns are missing, the same patient gets two numbers, and at month end nobody can say how many new patients came in.
This guide gives you a register format built for an Ayurveda OPD, a filled sample you can copy, and a free online OPD register you can print or download.

A general OPD register stops at the complaint and the fee. An Ayurveda OPD needs three more things:
Here is a sample day from a two-doctor Ayurveda clinic. All names and numbers are made up.
| Sr. | OPD No. | Reg. No. | Name | Age / Sex | N / O | Complaint | Diagnosis | Treatment / Advice | Doctor | Fee | Next visit |
|---|---|---|---|---|---|---|---|---|---|---|---|
| 1 | T-01 | AMP-0412 | Ramesh Patel | 52 / M | N | Knee pain, 3 months | Sandhigata Vata | Shamana; Janu Basti advised | Dr. Joshi | ₹300 | After 7 days |
| 2 | T-02 | AMP-0233 | Meena Shah | 34 / F | O | Acidity, burning | Amlapitta | Shamana continued | Dr. Joshi | ₹200 | After 15 days |

| 8 | T-08 | AMP-0091 | Mahesh Gupta | 57 / M | O | Sugar control review | Prameha | Shamana continued | Dr. Joshi | ₹200 | After 1 month |
The mobile and city column is left out of the sample to keep it readable. Keep it in your own register.
At the bottom of each day's page, write a short summary. It takes two minutes and saves hours at month end.
| Total OPD | 8 |
| New / Old | 4 / 4 |
| Male / Female | 4 / 4 |
| Children (under 12) | 1 |
| Panchakarma advised | 4 |
| Fees collected | ₹2,000 |
Add the daily summaries for the monthly OPD report: total visits, new patients, the most common diagnoses and the fees collected. Check that the fees total matches the cash and UPI received for the day.
Add each patient as they come in. The daily totals are calculated for you. Print the register, save it as a PDF, download it for Excel, or print a blank register to fill by hand.
If your clinic is preparing for NABH, read our NABH documentation requirements. If the waiting room is the bigger problem, see OPD queue management for Ayurveda clinics.
In Ayumanager PRO, every registration and visit is added to the OPD register automatically, with daily totals for new and old patients and fees. Try it free for 30 days, with no advance payment.
See how it works on the OPD management software page.
What is a daily OPD register?
A daily OPD register is the day-wise record of every patient seen in the out-patient department. Each row is one visit, with the OPD number, registration number, patient details, new or old, complaint, diagnosis, treatment, doctor and fee. At the end of the day the register is totalled for the daily OPD count and collection.
What columns should an OPD register have?
At minimum: serial number, OPD or token number, registration number (UHID), patient name, age and sex, mobile or city, new or old, chief complaint, diagnosis, treatment or advice, doctor, fee and receipt number, and next visit or remarks.
What is the difference between an OPD number and a registration number?
The registration number (UHID) is given once, on the patient's first visit, and stays the same for life. The OPD or token number changes on every visit and only identifies that day's visit. A register needs both, so you can count visits and still trace every visit back to one patient.
Can I keep the OPD register in Excel?
Yes. Use one sheet per month with one row per visit, and add the date as a column. Excel makes totals easy, but it does not stop duplicate registration numbers or missing fields, and the file must be backed up and protected because it holds patient information.
How long should OPD registers be kept?
Keep them for as long as the rules that apply to your clinic require, which can be several years. Check with your state's clinical establishment rules, your regulator, and NABH standards if you are accredited. Never destroy registers that relate to a complaint or legal case.
Does Ayumanager PRO create the OPD register automatically?
Yes. When the front desk registers a patient and the doctor completes the visit, the entry is added to the OPD register automatically, with the daily count and collection totals. You can view it by day, doctor or date range, and see the day's patients on the Android app.
